Upon Discharge from Hospital
Each new ostomy patient will receive a customized patient discharge kit. This kit can be shipped directly to the patient's home upon discharge. Each Discharge kit consists of:
- One box of pouches
- One box of wafers, including a stoma measuring guide
- Allkare barrier wipes
- Stomahesive paste
- Scissors to cut wafers to appropriate size
- Instructional video
- Carrying case
Patient Support
ConvaTec, serving as Better Living Now's preferred manufacture, offers patient support and counseling through their Professional Services Department. This department is made up of a full-time staff of fully trained professionals, including registered nurses. They are available at 800-422-8811 between 8:00 a.m. and 5:00 p.m. EST. The Professional Services team provides clinical and product information and support services for healthcare professionals as well as patients.
Patient Follow-up
During the second week at home the patient will receive a call from the ConvaTec Professional Services Department to ask "How are you doing and Can we help?"
With each re-order the patient will be asked if they are having any problem with their Ostomy. The BLN Care Coordinator will address these issues and if necessary they will be referred to the ConvaTec Professional Services Department. This follow-up will ensure that all eligible patients are being followed up on every three (3) months. All responses will be documented and reported to the health plan and\or the PCP.
Educational Material
The patient will also receive appropriate educational pamphlets. These pamphlets/videos are available in English and Spanish. With these pamphlets/videos the patient will also receive a laminated care which provides step by step instructions for removing and applying their appliance.
Value Added Patient Programs
Each new patient will be enrolled in the Better Together Club. The Better Together Club is a national organization organized by ConvaTec. The club's goal is to provide inspirational support, information on the latest ostomy-care products and techniques, plus gives the patient an opportunity to interact with others in similar circumstances. At no charge to the patient or health plan the members of this club will receive a quarterly newsletter.
Each patient will become eligible for the Great Comebacks Program. ConvaTec and the Crohn's and Colitis Foundation of America, Inc. (CCFA), created the Great Comebacks Program to honor people who have successfully overcome the difficulties of living with an ostomy or inflammatory bowel disease. Each year one winner is selected
