Patient Follow-up
During the second week at home the patient will receive a call from the ConvaTec Professional Services Department to ask "How are you doing and Can we help?"
With each re-order the patient will be asked if they are having any problem with their Urological. The BLN Care Coordinator will address these issues and if necessary they will be referred to the ConvaTec Professional Services Department. This follow-up will ensure that all eligible patients are being followed up on every three (3) months. All responses will be documented and reported to the health plan and\or the PCP.
Educational Material
The patient will also receive appropriate educational pamphlets. These pamphlets/videos are available in English and Spanish. With these pamphlets/videos the patient will also receive a laminated care which provides step by step instructions for removing and applying their appliance.
